FPPC Form 700

All Board members (Directors and Alternates) and some staff and vendors are required to file a Statement of Economic Interests Form 700 with CalTIP in accordance with CalTIP’s Conflict of Interest Code.

Members are required to file the following statements after being appointed to the CalTIP Board, before departure, and annually during continued service:

  • Assuming Office statement – to be filed within 30 days of assignment to the CalTIP Board
  • Annual Statements – to be filed by April 1st of each year (or the nearest weekday)
  • Leaving Office statement – to be filed within 30 days of departure from the CalTIP Board

Occurrence Report Form Packets

Packets for your agency vehicles are available and include: occurrence report form, courtesy cards, and pencils.

  1. Number of forms or packets needed;
  2. Name of person you want them sent to; and
  3. Physical address where they should be mailed.
