Report Incidents and Claims

To ensure timely handling, members must submit any incident or claim as soon as possible, using the corresponding form below.

Incidents/Occurrences

Submit any incident or occurrence involving Member vehicles in which a Tort Claim has not yet been filed with the Member

Claims

Submit a claim for any written Tort Claim from a third party or their attorney seeking money for bodily injury or property damage
